Purpose of the OODP Request Form
The purpose of the OODP Request Form is to allow organizations to request the services provided by the OODP.
Options for Submitting this Form
You have two options as to how to fill out the OODP Request Form:
- Fill out the online version below and click on the “Submit Form” button on the bottom of the page
- Download and print out a copy of the request form and mail it to the OODP (you can download a copy of the form from here)
Instructions for Online Version
Important notes for filling out the online request form:
- This online form cannot be saved. If your browser crashes or you cannot keep your browser open during the whole time it takes to fill out the form, you will lose all of your answers. For this reason, we recommend that you prepare your answers to the questions ahead of time and then fill in the form all at once. Minimizing the window will not affect your data.
- The online form is submitted via email to the OODP Administrator. A copy will be automatically mailed to the address specified by yourself in the “Organization Information” section of this form.
- All boxes must be filled in order for the Request Form to be processed. If certain parts of this form do not apply to your organization, please type “N/A” in the box.
